Fall Risk Assessment
Every resident undergoes a thorough fall risk assessment upon arrival, evaluating their mobility, strength, and medical history. These assessments help us create individualized care plans, which are regularly updated to adapt to each resident’s changing needs.
Balance & Strength Training
We offer balance and strength-building exercises to help seniors improve coordination, enhance stability, and restore confidence in their movement. For those participating in a rehabilitation program, therapy services are typically covered by insurance up to a certain point. For residents interested in additional wellness-based exercises outside of rehab, private pay options are available.
Mobility Assistance & Adaptive Equipment
We provide a variety of assistive tools—such as walkers, grab bars, and non-slip flooring—to support safe movement throughout our communities. Through our sister company, MedResources, residents at our Missouri, Kansas, Illinois, and Georgia locations also have access to durable medical equipment (DME) and additional assistive devices as needed.

Short-Term Rehabilitation After a Fall
If you or a loved one has experienced a fall and aren’t ready to make a permanent move to a facility, Delmar Gardens offers a short-term rehabilitation option designed to help you regain strength and independence. Our program provides expert therapy and 24/7 skilled nursing care in a safe, supportive environment, helping you rebuild mobility, restore confidence, and reduce the risk of future falls.
But we don’t stop there—before returning home, many of our residents have the opportunity to participate in a transitional stay at one of our supported independent living communities, located on the same campus as our rehabilitation centers (in most cases). This three-day, two-night trial stay offers all the comforts of home, along with access to medical staff at the push of a button, giving residents the chance to experience independent living with safety and support nearby.

From Humble Beginnings to Skilled Care Leaders
Skilled Nursing Care is the hallmark of the Delmar Gardens Family. Our late founders Barbara and Henry Grossberg, with their sons George and Gabe, provided care to elderly friends and neighbors out of their modest, University City, Missouri apartment. As popularity of their kindness grew, so did the need for space.
The Grossberg family met the Goldberg family in the early sixties and together in 1965, the first Delmar Gardens skilled nursing and rehabilitation opened. Today, the Grossberg and Goldberg families proudly continue exclusive ownership and operations of Delmar Gardens, ensuring family first…always.

If you’ve recently fallen, our short-term rehabilitation program can help you recover safely and confidently. Many of our residents stay with us for a short period before transitioning home—or, depending on your needs, you may want to explore Independent Living or Assisted Living options available at many of our rehabilitation locations.
