For years, St. Joseph Villa Homecare and Hospice has been enhancing the lives of patients and their families by combining the most advanced medical care with a focus on compassion and comfort. We are seeking a full time Hospice RN Case Manager for the Omaha area. As an RN Case Manager, you will:

  • Serve as the team leader of an interdisciplinary group of professionals who, together with the patient, develop a plan of care that best meets the needs of the patient and his/her loved one
  • Work with patients in the home or nursing facility setting
  • Attend a team meeting weekly where you work with the support of other professionals to problem solve and update the patient’s plan of care
  • Visit your patients 1-2 times a week on average to assess and coordinate care of the patient
  • Receive ongoing training and educational opportunities

The ideal candidate for this position is a dedicated professional with excellent assessment and organizational skills who prefers to work in the home/nursing home setting where care is provided in the patient’s own environment. Hospice, homecare, med/surg, oncology or long-term experience is preferred, as is computer proficiency. Current RN license and care insurance are required.
